Both parts must be completed and submitted to Eugene Burns by e-mail by May 15. Download the Portrait of Practice form in Microsoft Word format. 6) Complete the self-evaluation rubric and submit by e-mail to Eugene Burns by June 1. Download the self-evaluation rubric in Microsoft Excel format. 7) Prepare and submit a final financial report which will include the final budget form and an itemized statement of expenses. The itemized statement and the budget form must include institutional matching funds. This should be submitted by e-mail to Eugene Burns by July 20.
