    What is asthma?
    Asthma affects the small airways (bronchioles) that carry air in and out of the lungs. If you have asthma your airways can become inflamed, swollen and constricted (or narrowed) and excess mucus is produced. More than 5.2 million people in the UK are being treated for asthma and about 1.1 million of these are children. Asthma affects approximately one in 12 adults and one in eight children in the UK. This means there is a person with asthma in one in five households in the UK. It can affect almost anyone, at any age, anywhere although it tends to be worse in children and young adults. An asthma 'attack' describes the symptoms of tightness in the chest, a wheezing or whistling noise in the chest, coughing and difficulty breathing that occur when the airways become narrowed, inflamed and blocked by plugs of mucus.

Historic Contra-indication to diving
Traditionally, asthma has been thought to be an absolute contraindication to diving. The classical thought is that the asthmatic has air-trapping associated with the constrictive airway disease, bronchospasm and mucous plugging. Because of these factors -the vast majority of diving physicians felt that the risk for developing arterial gas embolisms to be great and that asthmatics should not be permitted to dive. In the recent past, workshops have been conducted and the consensus was that asthma should no longer be considered the absolute contraindication to diving that it was previously thought to be. Instead, the potential diver must be made aware that they are facing a relative risk of an event that occurs in less than 1 in 250,000 dives. Recommendations have been developed that include the following: 1). Exercise or cold induced asthmatics should not dive.(BS-AC adds 'emotional attacks') 2). Asthmatics requiring 'rescue or reliever' medication should not dive. Asthmatics on chronic maintenance bronchodilation ('controller') and inhaled steroids are thought to be able to dive. Recommendations vary, however, and the BS-AC recommends that asthmatics should not dive if he/she has needed a therapeutic bronchodilator in the last 48 hours or has had any other chest symptoms.They feel that the asthmatic should not need more than

e-mail rex.harrison@mac.com Despite huge improvements in the managment of asthma over the last 20 years approximately 1600 people die each year from asthma, in England and Wales alone, and most of these deaths occur outside hospital.( The hands are placed on either side of the lower chest and a firm but continuous pressure is exerted while the patient is breathing out. Timing of the chest compressions is made easier because respiration in asthmatic attacks is very noisy. The method should be used in conjunction with all the standard treatments for asthma and could easily be used by relatives and first-aiders before medical help arrives. For a more detailed explanation, click here This method came from my experience with three extremely unusual and dramatic cases, which occurred within a relatively short time. For further details see the Navigation Bar (left). All the patients had used Salbutamol aerosol inhalers and discarded them prior to my arrival. It is rare for respiratory arrest to occur in asthma; for a Doctor to arrive at the moment when a patient stops breathing in an asthmatic attack is extremely rare and not well documented. It happened to me twice in a short space of time. Despite the extreme rarity of these events they directed me to an important principle which requires further debate and assessment.

Public health importance of work-related asthma
Workplace exposures are responsible for the development of an estimated 10 - 20% of all new onset adult asthma cases. Asthma has become the most prevalent work-related lung disease in the United States. Work-related asthma is a serious and sometimes fatal disease. Workplace exposures to fumes, gases, vapors or dusts may aggravate asthma in a worker with preexisting asthma or may cause asthma for the first time in a previously healthy worker. Currently there are more than 350 substances used in the workplace that are known to cause asthma, including wood dusts, isocyanates, epoxies and certain adhesives. From 1995 to 2002, the Washington State workers' compensation claims rate for work-related asthma increased by 63%, from 7.9 claims per 100,000 full-time equivalents (FTE) to 12.9 claims per 100,000 FTEs. In July 2000, the public health importance of occupational asthma in the state of Washington was recognized when the State Board of Health made asthma a reportable condition.

Your child may have asthma if he or she often wheezes, coughs and has trouble breathing. These symptoms may get worse when your child gets sick, exercises or is exposed to certain triggers and irritants (see below).       Asthma: Overview
      Asthma is a chronic respiratory disease-sometimes worrisome and inconvenient-but a manageable condition. It is also known as reactive airway disease. With proper understanding, good medical care and monitoring, you can keep asthma under control. If you have asthma, you are not alone. In the United States, asthma affects 14 to 15 million people. Asthma, also known as reactive airway disease, is defined as a chronic lung condition with inflammation (swelling) of the airways, increased sensitivity of the airways to a variety of things that make asthma worse, and obstruction of airflow that is reversible. There are various type of asthma, including: 
      Typical Changes in the Airway
      Inflammation
      Recent research has shown that inflammation of the lining of the airways is the most common feature of asthma. When they are stimulated, certain cells lining the airways release chemical substances (mediators) that lead to inflammation. This causes the airway lining to swell and narrow. The inflammation may last for hours, days or weeks following an episode. Most people with asthma have some degree of inflammation all of the time. Some long-term control medications can help prevent and reduce inflammation.

Asthma in Australian children: findings from Growing up in Australia , the Longitudinal Study of Australian Children
Within the first three years of life, 16.9% of infants experience asthma or wheeze. Among non-asthmatic children aged 4 to 5 years, 4.1% will develop asthma by the 7th year of life.

Burden of disease due to asthma in Australia 2003
In 2003 asthma was the leading cause of burden of disease in Australian children, contributing 17.4% of total DALYs and the eleventh-leading contributor to the overall burden of disease in Australia, accounting for 2.4% of the total number of DALYs. Asthma also contributed a substantial proportion to the Indigenous health gap in the burden of non-communicable conditions. It is predicted that asthma will continue to rank as one of the major causes of disease burden in Australia for the next 2 decades, particularly among females.
