  • Low-potency topical agents, such as 1% hydrocortisone or desonide:

    • Can be used in more severe cases on the trunk and extremities
    Strong, halogenated steroids:
    • Should not be applied to the face, axillae, or groin Oral systemic steroid taper should rarely be prescribed.
    Most worrisome side effects
    • Dermal and epidermal atrophy Suppression of the pituitary-adrenal axis Applying topical steroids to the occluded groin area of a diapered child increases the risk of systemic side effects.
    Nonsteroidal immunomodulators and other topical medications
    • Topical tacrolimus and pimecrolimus
      • Most efficacious in mild to moderate disease Alternatives to topical steroids Can be applied to the face Theoretical risk of cancer Other topical medications, coal tar and doxepin, are not routinely prescribed by pediatricians.

Pain management specialists, like neurosurgeons, are best equipped to treat pain. Pain specialists use traditional and alternative treatments, medications, injections, exercises, counseling, acupuncture and whatever else is needed to relieve suffering. Pain is difficult to describe but is immediately obvious. It can be hot, sharp, dull, aching, or heavy. It can be slight or overwhelming. Pain usually warns us when something is wrong. When it does not go away it can rob us of our productivity, well-being, and our very lives. Pain is perceived differently by every patient. In 1931, Dr. Albert Schweitzer called pain "a more terrible lord of mankind than even death itself." The International Association for the Study of Pain defines pain as: "An unpleasant sensory and emotional experience associated with actual or potential tissue damage or described in terms of such damage." Pain has become increasingly common and costly. It is a challenge for family, friends, and the doctors who care for pain sufferers.

Asperger Syndrome by Vera Bernard-Opitz, Kenneth Lyen, Lam Chee Meng Definition Asperger syndrome is defined as a condition resembling autism in which there are speech and language abnormalities, problems with nonverbal communication, severe impairment in social interaction, a fixation on a narrow field of interest, rigidity in following unchangeable routines and motor clumsiness. Historical In 1944, Hans Asperger described a distinctive pattern of behavior that resembled autism and is now referred to as Asperger syndrome. A year earlier, Kanner had published his observations on autism. Unlike Asperger syndrome in which the intelligence is normal or high, Kanner’s autistic subjects were quite severely intellectually disabled. Epidemiology In Sweden, the prevalence of Asperger syndrome is around two per 1,000 school children, and boys outnumber girls by quite a large margin. Asperger syndrome is more common and less severe than Kanner autism. Inheritance A number of families have a mix of members with Asperger syndrome and Kanner autism. This suggests that there is an overlap in these two conditions, and it may eventually turn out that they actually represent different degrees of severity of the same condition.

  • Treatment of uncomplicated cases
    • Treatment to relieve dryness, inflammation, and pruritus and eradicate secondary bacterial infections Daily applications of emollients (cream or ointment), especially after brief warm baths
      • Ointments are better than emollients in more severe cases. If discomfort (stinging sensation) occurs or if ointment feels too occlusive (eg, in humid weather), creams can be substituted.
      Bathing should last no longer than 5 minutes.
      • Mild soaps or nondetergent cleansers are recommended. Pat dry with a towel and apply emollient immediately to all skin. Apply prescription topical medicines to affected areas.
      Inflammation is best treated with corticosteroids.
      • Generally, ointments tend to work better than creams.

This is in response to the letter from Kristin Brewer in the July 2006 issue. She wanted help with her cat Simon who attacks only her. I had the same problem with one of my cats (I had five). I was the only one in a family of four who was Teddy's target. I discovered that he only attacked when I used a particular brand of soap (in my case, it was Ivory). Ms. Brewer might try changing her soap, lotion, shampoo or perfume. That may help. JoAnn Davis
