Borderline Personality Disorder
A Profile by Paul J. Hannig, Ph.D., MFCC, CCMHC, NCC This is an excerpt from the full length article, Borderline Personality Disorder (BP): A Profile. It is written to include only the pathological aspects of the disorder with full recognition that healthy aspects of behavior do exist. According to the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (American Psychiatric Association, 1994) "the essential feature of Borderline Personality Disorder is a pervasive pattern of instability of interpersonal relationships, self-image, and affects and marked impulsivity that begins by early adulthood and is present in a variety of contexts." The Borderline feels eternally alone and abandoned. Intense abandonment fears and inappropriate anger are experienced when they are faced with separation or even an unavoidable change of plans. Consequently, when the impaired self is activated due to family frustrations, stress, etc., the BP will lose and withdraw feelings of love for a significant other. They will experience intense abandonment fears and inappropriate anger when faced with the threat of separation (real or unreal) or even an unavoidable change of plans. If there has been a death or some other loss, the BP looks to replace the lost nurturance and sustenance. Hence, borderline patterns of relationships are unstable and intense. If a BP feels that the other person in the relationship does not care enough, they may quickly switch from idealization to devaluation. The unspoken expectation of the borderline is for others to "be there" on demand at all times. Temporary commitment, withdrawal and the search for a perfect, all loving, non-exploitive love object is the continuous and dysfunctional emotional cycle of the borderline.

Borderline Personality Disorder
Borderline Personality Disorder (BPD) is one of the most controversial diagnoses in psychology today. Since it was first introduced in the DSM, psychologists and psychiatrists have been trying to give the somewhat amorphous concepts behind BPD a concrete form. Kernberg's explication of what he calls Borderline Personality Organization is the most general, while Gunderson, though a psychoanalyst, is considered by many to have taken the most scientific approach to defining BPD. The Diagnostic Interview for Borderlines and the DIB-Revised were developed from research done by Gunderson, Kolb, and Zanarini. Finally, there is the "official" DSM-IV definition. Some researchers, like Judith Herman, believe that BPD is a name given to a particular manifestation of post-traumatic stress disorder: in Trauma and Recovery, she theorizes that when PTSD takes a form that emphasizes heavily its elements of identity and relationship disturbance, it gets called BPD; when the somatic (body) elements are emphasized, it gets called hysteria, and when the dissociative/deformation of consciousness elements are the focus, it gets called DID/MPD. Others believe that the term "borderline personality" has been so misunderstood and misused that trying to refine it is pointless and suggest instead simply scrapping the term.

Writing on the bus.
I know, I have been a terrible blogger as of late. I promise to write more and I don't... Well, that's in part because I vent my emotions to Ross so it sometimes seems redundant if I do it here, too. The other reason is that I want to redesign this blog but I haven't been able to procure a copy of Photoshop for my Macboo yet. The (illegal) version on my old Dell is useless because the computer can't handle it.
So, here I am, writing this on my smartphone (yay Moto Q!) because reading books and writing on paper gives me a headache while I ride but this doesn't. Huh. I'm sure it is just all in my head, as usual.
I'm on my way to see my psychiatrist and, as of today, my new employer. Dr. Hardrict is perhaps the kindest person I've ever met (aside from Ross, though I guess I'm biased). When I expressed my concerns about finding a job because my credentials and professional references were lacking, considering the few jobs there are probably have a bajillion applicants, he said, "Well then, what do you think about working for me?"
My jaw dropped. I had to ask if he was really serious and he said, "Of course, I could really use the help." I knew he'd been running his small, private office by himself since his last secretary/office worker left, but I'd supposed than he didn,t need as much help since he cut back on his client load there. He said he could really use the help and that it could be incredibly beneficial for us both.

Borderline Personality Disorder Symptoms and signs
The clinical presentation of borderline personality disorder varies widely and can be very vague. Common features include:
  • somatic complaints such as headaches, abdominal pain or malaise; autonomic instability as indicated by cold hands, nausea, vomiting, diarrhea; a tendency to demonstrate factitious illnesses, which may lead to overuse of medical resources; a pattern of undermining themselves just as a goal is about to be achieved; psychotic-like features, especially in times of stress; a history of suicide attempts or self-mutilation; a history of a history of substance misuse.
Borderline personality disorder is usually accompanied by one or several `symptom disorders', such as an eating disorder, depression, post-traumatic stress disorder, premenstrual tension, dissociative disorder or anxiety disorder.
Investigations
A full psychiatric history should be obtained.

According to the latest BPD research, borderline personality disorder has a lifetime prevalence of up to 5.9% in our population. Because systematic research has only relatively recently been initiated, BPD is at least two decades behind in research, treatment options and family education compared to other major psychiatric disorders. The core disorder in BPD is emotional dysregulation. Emotional dysregulation is viewed as a joint outcome of biological disposition, environmental context, and the transaction between the two during development. The theory asserts that borderline individuals have difficulties in regulating several, if not all, emotions. This systemic dysregulation is produced by emotional vulnerability and by maladaptive and inadequate emotion modulation strategies. The symptoms of borderline patients are similar to those for which most people seek psychiatric help: depression, mood swings, the use and abuse of drugs and alcohol as a means of trying to feel better; obsessions, phobias, feelings of emptiness and loneliness, inability to tolerate being alone, problems about eating.
The one word that best characterizes borderline personality is "instability." Their emotions are unstable, fluctuating wildly for no discernible reason. Their thinking is unstablerational and clear at times, quite psychotic at other times. Their behavior is unstable, often with periods of excellent conduct, high efficiency and trustworthiness alternating with outbreaks of babyishness, suddenly quitting a job, withdrawing into isolation, failing.
