Borderline personality disorder
Borderline personality disorder is a condition in which a person makes impulsive actions, and has an unstable mood and chaotic relationships.
Causes
Personality disorders are long-term (chronic) patterns of behavior that negatively affect relationships and work. The cause of borderline personality disorder (BPD) is unknown. People with BPD are impulsive in areas that have a potential for self-harm, such as drug use, drinking, and other risk-taking behaviors. Risk factors for BPD include:
  • Abandonment in childhood or adolescence Disrupted family life Poor communication in the family Sexual abuse
This personality disorder tends to occur more often in women and among hospitalized psychiatric patients.
Symptoms
Relationships with others are intense and unstable. They swing wildly from love to hate and back again. People with BPD will frantically try to avoid real or imagined abandonment. BPD patients may also be uncertain about their identity or self-image. They tend to see things in terms of extremes, either all good or all bad. They also typically view themselves as victims of circumstance and take little responsibility for themselves or their problems.

parasuicide (Kreitman, 1977). Self-mutilation is an especially prevalent and chronic problem among those diagnosed with borderline personality disorder (BPD). BPD is a chronic debilitating disorder that is characterized by pervasive dysfunctions in emotions, behavior, and cognition. More BPD individuals engage in self-mutilation and attempt suicide than other diagnostic groups ( Although behavior therapy has been used for many years to treat many different psychological disorders, it has only recently been applied to the treatment of self-mutilation. Several studies suggest that cognitive behavior therapy can reduce suicide attempts, but I could locate only two randomized controlled treatment studies that may be relevant to self-mutilation. Linehan et al. (1991) found Dialectical Behavior Therapy (DBT) to be effective in reducing parasuicidal behaviors overall. The results of another study suggest that a manual-assisted short-term cognitive behavior therapy may reduce parasuicide (Evans et al., 1999). Neither study, however, reported outcomes specifically for self-mutilation. This paper will present a behavioral conceptualization of parasuicide based on Linehan's biosocial theory of BPD (1993). Next, behavioral treatment strategies will be reviewed that are used to treat parasuicidal behaviors in DBT, a comprehensive treatment for BPD and parasuicidal behaviors based on this biosocial theory and social learning theory (Linehan, 1993). Although this paper will focus on self-mutilation, the etiology and treatment of both forms of parasuicide will be discussed. Although self-mutilation and suicidality are not the same, these two forms of parasuicide often co-occur in BPD, and it is likely that reducing the vulnerability to one can also lead to improvements in the other.

      WHAT IS BORDERLINE PERSONALITY DISORDER? Borderline personality disorder is characterized by emotion dysregulation, meaning quick, frequent, and painful mood swings that are beyond the control of the person with the problem. People struggling with this problem have great difficulty forming and maintaining relationships. They also experience problems controlling their own spontaneous and reckless behaviors and often have a fluctuating idea about who they are. The overall theme for this disorder is rapid and unpredictable changes in a person’s thoughts, moods, behaviors, relationships, and beliefs. Very often, these rapid changes are caused by recurring fears of being criticized or deserted by other people, or they are triggered by actions of other people that feel like criticism, such as small disagreements or changes in plans. In response to these types of situations, a person with borderline personality disorder can suddenly become very sad, nervous, angry, or short-tempered. The person might also practice self-harming behaviors, like cutting himself or herself, or engage in suicidal acts. Unfortunately, personality styles like this often create problems in a person’s relationships, job, and other social situations, which is why they’re referred to as personality disorders.

    What is Borderline Personality Disorder?

    Additional Information:
    Borderline personality disorder was so-named because it was originally thought to be at the "borderline" of psychosis. The disorder is relatively common, affecting 2% of adults. Women are much more likely to suffer borderline than men. Nearly 20% of psychiatric hospitalizations are due to borderline. With treatment, patients are often able to see their symptoms improve.
    Treatment involves therapy in which the patient learns to talk through his or her feelings rather than unleashing them in destructive and self-defeating ways. Medication may be helpful, and treatment of any alcohol or substance abuse issues is required. Brief hospitalization is sometimes required, especially in cases involving psychotic episodes or suicide threats or attempts.

There, I came across behavior I could not ascribe to bipolar disorder – extremely abusive verbal attacks, explosive meltdowns, public outbursts, poison pen emails, delusional self-centeredness, love and light one minute-on their shit list the next. Yes, bipolars can behave badly, but this was different. It was like being back in high school again, only worse. It got so I started to believe that the B in the acronym for our organization stood for something other than bipolar. For my own self-preservation, I got out of this toxic environment and cut off all ties with the state organization. I literally wound up hating these individuals and their illness. But I also recognized how lonely life must be for these individuals. None of them was married or in a loving relationship. None of them had children. None of them was employed. All of them engaged in frightening behavior. All were serious accidents waiting to happen. The illness is called borderline personality disorder. On the surface, the emotional volatility, impulsivity, depressions, mood swings, high drama, and destructive behavior of individuals with this diagnosis resemble bipolar disorder. The suicide rate is in the bipolar ballpark, about ten percent, and the pain and suffering individuals with this illness experience is similar.

Comorbidity is common; borderline personality disorder frequently occurring with substance use disorders and affective disorders. Suicidality and completed suicide are possible outcomes without proper care and effective therapy. Diagnosis Diagnosis is based on the self-reported experiences of the patient, as well as markers for the disorder observed by a psychiatrist, psychologist, or other qualified diagnostician through thorough clinical assessment. This profile may be supported and/or corroborated by long term patterns of behavior as reported by family members, friends or co-workers. The list of criteria that must be met for diagnosis is outlined in the DSM-IV-TR.[1] An initial assessment generally includes a comprehensive personal and family history, and may also include a physical examination by a physician. Although there are no physiological tests that confirm borderline personality disorder, medical tests may be employed to exclude any co-occurring medical conditions that may present with psychiatric symptoms. These include blood tests measuring TSH to exclude hypo- or hyperthyroidism, basic electrolytes and serum calcium to rule out a metabolic disturbance, full blood count including ESR to rule out a systemic infection or chronic disease, and serology to exclude syphilis or HIV infection; two commonly ordered investigations are EEG to exclude epilepsy, and a CT scan of the head to exclude brain lesions.
