We treated and discharged 76 patients. Our average length of stay was 22.62 days. The national average was 16.43 days. The regional average was 16.59 days. Our average FIM score change from admission to discharge was 23.13 points. The national average was 23.97 points. The regional average was 23.13 points. Our average FIM total discharge score was 74.03 points. The national average was 76.68 points. The regional average was 74.72 points.
The Brain Injury Program at National Rehabilitation Hospital is the busiest in the Washington, DC area, treating more than 200 patients every year. The mission of the National Resource Center for Traumatic Brain Injury (NRCTBI) is to provide relevant, practical information for professionals, persons with brain injury, and family members. We have more than two decades of experience investigating the special needs and problems of people with brain injury and their families. With input from consumers and nationally recognized experts, we have developed a wide variety of assessment tools, intervention programs, and training programs. The NRCTBI is housed at Virginia Commonwealth University’s Medical College of Virginia Campus. Many of our staff are affiliated with the Virginia Commonwealth Traumatic Brain Injury Model System. HOW CAN THE BRAIN BE INJURED?
In the United States traumatic brain injury (TBI) is a leading cause of death for persons under age 45. TBI occurs every 15 seconds. Approximately 5 million Americans currently suffer some form of TBI disability. The leading causes of TBI are motor vehicle accidents, falls, and sports injuries. While the brain is by far the most complex object on earth, it is soft and vulnerable with a consistency of firm pudding. A concussion is a sudden trauma-induced alteration of the alert state. The person may be unable to concentrate or be confused for a few seconds, or completely lose consciousness and fall down. The brain is capable of recovering from a concussion. How much force is necessary to cause permanent brain damage is under study, and hence still unclear. Over the years, professional boxers suffer permanent brain damage. The force of a professional boxer's fist is equivalent to being hit with a 13 pound bowling ball traveling 20 miles per hour, about 52 g's. Plopping down into an easy chair can generate up to 10 g's. So, it seems that somewhere between 10 and 50 g's is the threshold to permanent brain injury. This does not mean that accelerations over 50 g's have to cause permanent brain damage. Football players are subjected to 200 g's, and Indy race car drivers have been subjected to 80 g's without permanent injury, but they were wearing helmets.

Overview of TBI

Although any injury to the brain is serious, and severe damage can be fatal, medical and surgical advances have improved the odds for surviving a TBI. Nevertheless, coping with the life-changing consequences of traumatic brain injury presents a great challenge for patients, families, physicians, therapists, and society.
Incidence and Prevalence of TBI
According to the Centers for Disease Control and Prevention (CDC), approximately 1.4 million people suffer TBI each year in the United States and about 50,000 people die from the injury. Estimates of the number of people who have survived a TBI range from 2.5 million to 6.5 million. The range is broad because mild TBI often goes unreported. The cost of traumatic brain injuries in the United States is estimated at $48.3 billion annually: $31.7 billion in hospitalization costs and another $16.6 billion in costs associated with fatalities.

Nearly all of the survivors of a traumatic head injury and their families with whom I have worked have had one complaint: There is nothing written that explains head injury in clear, easy to understand language. Most say the available material is too medical or too difficult to read. The goal of this online book is to better prepare the head injured person and family for the long road ahead. See our Table of Contents below. PUBLICATION HISTORY
This online book came from materials that I wrote for my patients and their family members. I basically used a copy machine and gave copies away for free. This book became very popular and over a thousand copies have been given out. It costs about $6 to copy the book another $3 to $4 to mail to people. The costs and time to send the books got out of hand. I decided not to publish the book. Few people would have access to the material and I would not be able to "update" or improve the material easily. Using the Internet, anyone with a computer could access this information. It is my sincere hope that the Internet version of my book would help the greatest number of people. Go to each chapter heading and click on it. Use your web browser to print any information that you find helpful. There is no charge for the information you copy.

Introduction
Traumatic brain injury (TBI) is a major public health problem, especially among male adolescents and young adults ages 15 to 24, and among elderly people of both sexes 75 years and older. Children aged 5 and younger are also at high risk for TBI. This computer-generated graphic shows how, in 1848, a 3-foot long, pointed rod penetrated the skull of Phineas Gage, a railway construction foreman. The rod entered at the top of his head, passed through his brain, and exited his skull by his temple. Gage survived the accident but suffered lasting personality and behavioral problems. Today, we understand a great deal more about the healthy brain and its response to trauma, although science still has much to learn about how to reverse damage resulting from head injuries.
