Dengue is the fastest emerging arboviral infection. The maximum burden is borne by countries of the Asia Pacific Region. Among the estimated 2.5 billion people at risk globally, about 1.8 billion—more than 70%—reside in Asia Pacific countries. Recent estimate made by Pediatric Dengue Vaccine Initiative however showed that population at risk is about 3.6 billion people (55% of world population) in 124 endemic countries . Its epidemiology is rapidly evolving, with increased frequency of outbreaks and expansion to new geographical areas that were previously unaffected. Mortality is highest during the initial period of the outbreak or epidemic. Children in particular are at high risk of mortality as a result of complications and lack of access to prompt treatment.

Reports to WPRO showed that continues its increasing trend. Few years after the large dengue outbreaks in 1998, Member States in the Western Pacific started to report increased number of dengue cases. Annual reported dengue cases were around 100 000 cases in 2001-2002. The figures gradually increased to 150 000-170 000 cases annually during the period 2003-2006. Since 2007, the region reported over 200,000 dengue cases each year. In 2009, there were 242, 424 dengue cases and 785 dengue deaths were reported in 25 out of 37 countries and territories in the region. Countries that were hardest hit include:

Somewhere in Tanzania, maybe it was in Uganda, a lucky mosquito found its prey, zoomed in on its target and stole some blood from an unlucky human. This bite began innocently enough - happening during the day, not causing much of an itch - but several days later it lost all pretense of innocence. Lethargy, an unusual amount of tiredness, was the first sign that something was going awry. It was soon followed by a headache behind the eyes that throbbed and pounded, with a sensation of pressure like a kettle brewing and boiling. A fever, mild at first, but later intense with sweating, came bundled with ferocious muscle aches. These aches were rooted deep in the calves and back, and felt like being punched from the inside-out. The once-dinner-for-a-stray mosquito became apathetic and lost all appetite. What on earth could this be? Malaria? Typhoid? What, what, what? A funny pink rash showed up soon after. It didn't itch, it didn't bleed, it didn't hurt. It just spread - over the chest, belly, and back. It, along with the fever, lightened after two days but just two days later returned with even greater dramatic force. Ah, I think we have enough clues and a diagnosis now...

Dengue Fever Dengue ( déng gee, déng gày) is a mosquito-borne infection which in recent years has become a major international public health concern. Dengue is found in tropical and sub-tropical regions around the world, predominately in urban and peri-urban areas. Dengue haemorrhagic fever (DHF), a potentially lethal complication, was first recognized during the 1950s and is today a leading cause of childhood mortality in several Asian countries. There are four distinct, but closely related, viruses which cause dengue. Recovery from infection by one provides lifelong immunity against that serotype but confers only partial and transient protection against subsequent infection by the other three. Indeed, there is good evidence that sequential infection increases the risk of more serious disease resulting in DHF. Prevalence Some other statistics: An estimated 500 000 cases of DHF require hospitalisation each year, of whom a very large proportion are children and roughly 5% die. Without proper treatment, DHF case fatality rates can exceed 20%. With modern intensive supportive therapy, can be reduced to less than 1%.

L’interrogatoire apprend qu’il vient de faire un séjour touristique de deux mois à la Martinique et qu’il est rentré depuis 4 jours en France métropolitaine. Au milieu de son séjour, il a passé une semaine en Guyane française. Quarante-huit heures après son retour de Martinique est survenu un syndrome pseudo-grippal avec embarras gastrique fébrile, douleurs diffuses, en particulier douleurs lors des mouvements des yeux. A l’examen clinique, on note une hyperémie conjonctivale avec un faciès vultueux, un exanthème fait de macules siégeant au niveau du tronc et des extrémités des membres, avec quelques pétéchies au niveau des pieds. Il n’y a ni adénomégalie, ni hépatomégalie, ni splénomégalie. La conscience est normale. Il n’y a pas de syndrome méningé. La tension artérielle est à 130/80 mmHg, le pouls à 96/mn. Il a été vacciné, avant son départ, contre les hépatites A et B, la fièvre typhoïde et la fièvre jaune. Il a suivi une chimioprophylaxie antipalustre par LARIAM

Natural History
The normal cycle of dengue infection is considered to be human - mosquito - human. From feeding on an infected and viraemic human, the female mosquito is able to transmit the dengue virus after an incubation period of 8-10 days wherein virus infection, replication and dissemination result in infection of the salivary glands making the mosquito infective for life. In Australia epidemics of dengue occurred in the late 19th century and early 20th century. Australia was considered to be free of local dengue following 1955 (when there had been a large outbreak in Townsville), but in 1981 a major outbreak with an estimated 3,000 infections occurred in northern Queensland, presumably initiated by an infected traveller. Causes
Four different dengue viruses are known to cause dengue hemorrhagic fever. Dengue hemorrhagic fever occurs when a person catches a different type dengue virus after being infected by another one sometime before. Prior immunity to a different dengue virus type plays an important role in this severe disease. Worldwide, more than 100 million cases of dengue fever occur every year. A small number of these develop into dengue hemorrhagic fever. Most infections in the United States are brought in from other countries. It is possible for a traveler who has returned to the United States to pass the infection to someone who has not traveled. Risk factors for dengue hemorrhagic fever include having antibodies to dengue virus from prior infection and being younger than 12, female, or Caucasian.
