The earliest population-wide genetic tests were performed on newborns in the 1960s and involved screening for the metabolic disorder phenylketonuria (PKU). Detection of this disease at birth allowed for early treatment. In most cases, mental retardation (a condition resulting from PKU) was avoidable. Testing of newborns, coupled with educational campaigns, can even result in eliminating some diseases associated with certain ethnic groups.

    DNA Testing
    Genetic fingerprinting or DNA profiling is a techniques used to distinguish between individuals using only samples of their DNA. It was invented by Sir Alec Jeffreys in 1985. Two humans will have the vast majority of their DNA sequence in common. Genetic fingerprinting exploits highly variable repeating sequences called minisatellites. Two unrelated humans will be unlikely to have the same numbers of minisatellites at a given locus. In STR profiling, which is distinct from DNA fingerprinting, PCR is used to obtain enough DNA to then detect the number of repeats at several loci. It is possible to establish a match that is extremely unlikely to have arisen by coincidence, except in the case of identical twins, who will have identical genetic profiles. Genetic fingerprinting is also used in such applications as identifying human remains, paternity testing, matching organ donors, and also been used to generate hypotheses on the pattern of the human diaspora in prehistoric times. PCR Analysis The polymerase chain reaction (PCR) is a biochemistry and molecular biology technique for isolating and exponentially amplifying a fragment of DNA, via enzymatic replication, without using a living organism (such as E. coli or yeast). As PCR is an in vitro technique, it can be performed without restrictions on the form of DNA, and it can be extensively modified to perform a wide array of genetic manipulations. Invented in 1983 by Kary Mullis, PCR is now a common technique used in medical and biological research labs for a variety of tasks, such as the sequencing of genes and the diagnosis of hereditary diseases, the identification of genetic fingerprints which is used in DNA Paternity Testing.

    • Introduction Hereditary hemochromatosis (HH) is regarded as the most common genetic disorder in Caucasians, with an estimated prevalence of 1/200-1/400 (homozygotes) and a carrier frequency (heterozygotes) of 1/8-1/10. It is an excellent example of a common genetic disorder with high morbidity and mortality which is preventable if diagnosed early. A DNA test for hemochromatosis has recently become available and will be a valuable aid for early detection of this disorder. Hemochromatosis is characterized by increased iron absorption in the intestine, resulting in a gradual build-up of excess iron (iron overload). Most homozygotes develop symptoms in adulthood, although the age of onset has been as young as 2 years. Heterozygotes are usually asymptomatic. Early symptoms in homozygotes are multisystemic and often non-specific, including malaise, fatigue, impotence, abdominal pain, or joint pain. As iron storage increases, deposits occur in organs such as the liver, heart, and pancreas, leading to organ damage and dysfunction.

    DNA-based tests are among the first commercial medical applications of the new genetic discoveries. Gene tests can be used to diagnose and confirm disease, even in asymptomatic individuals; provide prognostic information about the course of disease; and, with varying degrees of accuracy, predict the risk of future disease in healthy individuals or their progeny. Currently, several hundred genetic tests are in clinical use, with many more under development, and their numbers and varieties are expected to increase rapidly over the next decade. Most current tests detect mutations associated with rare genetic disorders that follow Mendelian inheritance patterns. These include myotonic and Duchenne muscular dystrophies, cystic fibrosis, neurofibromatosis type 1, sickle cell anemia, and Huntington’s disease.
