PHYSIOLOGY OF HEADACHES AND MIGRAINES
Headache, or Cephalgia, is defined as pain in the head that is not confined
to any one specific nerve distribution. This means that headaches can encompass a large area of the head and can affect many nerve areas simultaneously.
Generally, headaches can be divided into two basic groups, primary and secondary. Primary headaches are headaches that have no consistent organic cause. Secondary headaches are headaches that have an identifiable source and occur secondarily to other disease processes such as, being secondary to high blood pressure or to sinusitis.
Primary headaches can be further divided into three basic types. Most of us are familiar with 2 of them, they are Tension headaches, Migraine headaches and the third being Cluster headaches.

The exact genetic predisposition to this subtype of migraine headache is unknown. The familial occurrence is similar to that expected in all patients with migraine preceded by visual aura, with an estimated 25% positive familial history. Retinal migraine is expected to be more common in women than men, which is true of migraine headaches in general, but this also has not be documented.
One may consider two forms of anterior visual pathway migraine: (1) transient monocular blindness and (2) permanent unilateral visual loss, a much less common occurrence.

Ophthalmic Migraine
Ophthalmic migraine is quite common. Patients usually experience visual symptoms of seeing bright zig-zag type lines in their central or peripheral (side) vision. These bright lines may have associated flashing light sensations and sometimes can interfere with vision. There are many variations of symptoms. These symptoms usually resolve spontaneously after several minutes but usually less than one hour. Often, rest in a darkened room can be helpful during a migraine attack. Medical treatment is usually not necessary. Sometimes, there can be a headache after the visual symptoms resolve. This is called migraine headache with visual prodrome. Some people get migraine headaches without the visual symptoms. Some people get the visual symptoms without the headache. This is called Ophthalmic Migraine. The cause is due to a temporary spasm in the blood vessels behind the eye called "vasospasm" similar to a spasm or cramp that you may have once experienced in one of your leg muscles (Charlie-Horse). Usually, this resolves without treatment and many people never have another episode. Some people may continue to have them. An examination of the eye is important to rule out any other causes for these symptoms.
