Malnutrition may be due to inadequate food absorption or inadequate food intake (inadequate supply, increased requirements). The diagnosis of malnutrition is not always apparent as in severe cases. It depends on an accurate dietary history, evaluation of height, weight, head circumference and past rates of growth, measurement of midarm circumference and skinfold thickness and other tests. Acute nutritional disturbances: water and electrolytes. Chronic malnutrition: deficits of more than a single nutrient. There is usually associated immunologic insufficiency (white blood cell count < 1500/mm3 and anergy to skin test antigens).
Marasmus (Infantile Atrophy, Inanition, Athrepsia)
In most cases, marasmus is due to inadequate caloric intake, but may also be due to metabolic abnormalities or congenital malformations. Clinically, there is failure to gain weight followed by weight loss and finally emaciation. Fat is loss last from the cheeks. The abdomen may be flat or distended. There is muscle atrophy and hypotonia. The basal metabolic rate is reduced. The infant may be constipated or have the "starvation type" of diarrhea with mucus.

Malnutrition is a medical condition experienced by the body due to insufficient intake of nutrients required for normal and sustaining bodily function. There are many reasons why someone may be suffering from malnutrition. It could be a lack of knowledge regarding proper diet, that can lead a person to actually eat enough food, but not the right foods and therefore required nutrients. Poor absorption power of the body can also be a reason for malnutrition.
According to modern researchers the word malnutrition includes the concept of “over nutrition” as well. It’s not a healthy habit to gorge on food with only particular nutrients, as this practice can cause obesity and many other diseases. “Under nutrition”, such as an elongated period of starvation, will result in a severe lack of food-based nutrients. This leads the body to become much more vulnerable to many kinds of infections and diseases.
Poverty is among the most common reasons for malnutrition, and is an especially difficult problem for economically under-developed countries to counter. Lack of proper nutrition does not only affect one's body, but it can reduce the intelligence levels of a person. Famine has been found to be among the most common reasons behind the initiation of malnutrition among a particular group of people in a particular region. Famine leads specifically to a severe lack of requisite amounts of vitamins, protein, and essential minerals in the broad population’s diet. Medical problems occurring as the result of malnutrition are called deficiency diseases. Scurvy is at the top of the list of such diseases; affected persons lack enough Vitamin C.

In The State of the World's Children 1998, UNICEF urges intensive efforts by governments to counter the scourge, which it also regards as a violation of children's rights. The statistics on malnutrition are grim. Nearly 12 million children under the age of five (over 4 million of them in sub-Saharan Africa alone) die annually; malnutrition is a culprit in 55 per cent of these deaths. Iron deficiency anaemia is a contributing factor in over 20 per cent of post-birth maternal deaths in Africa and Asia. About 43 million people worldwide are suffering from varying degrees of brain damage due to iodine deficiency. Some 226 million children are stunted (shorter than they should be for their age); nearly 67 million are estimated to be wasted (weigh less than they should for their height); and about 183 million weigh less than they should for their age. Kwashiorkor is the edematous form of protein-energy malnutrition. It is associated with extreme poverty in developing countries and with chronic malabsorptive conditions such as cystic fibrosis in developed countries. Rare cases of kwashiorkor in affluent countries unrelated to chronic illness have been reported. We present 12 cases of kwashiorkor unrelated to chronic illness seen over 9 years by pediatric dermatologists throughout the United States, and discuss common causative themes in this easily preventable condition. Observations
Conclusions
Physicians should consider the diagnosis of kwashiorkor in children with perceived milk allergies resulting in frequent dietary manipulations, in children following fad or unorthodox diets, or in children living in homes with significant social chaos. The presence of edema and "flaky paint" dermatitis should prompt a careful dietary investigation.
