    zSB(3,3) What is Oppositional Defiant Disorder (ODD) ODD is a psychiatric disorder affecting behavior. Three characteristics of the child who has ODD are: aggression, defiance and the constant need to irritate others. When documenting the child's behavior; characteristics or behavior patterns should be in place for at least 6 months. The behaviors will have a negative impact on social and academic functioning. It is important to look for the following characteristics:
    • The child often loses his/her temper The child is defiant and doesn't obey rules/routines The child argues often with adults and peers The child seems to go out of his/her way to annoy others in very bothersome ways The child is often lacking accountability and blames others for inappropriate behaviors The child often seems angry, spiteful and vindictive

    Oppositional defiant disorder
    Oppositional defiant disorder is a pattern of disobedient, hostile, and defiant behavior toward authority figures.
    Causes
    This disorder is more common in boys than in girls. Some studies have shown that it affects 20% of school-age children. However, most experts believe this figure is high due to changing definitions of normal childhood behavior, and possible racial, cultural, and gender biases. This behavior typically starts by age 8, but it may start as early as the preschool years. This disorder is thought to be caused by a combination of biological, psychological, and social factors.
    Symptoms
    • Actively does not follow adults' requests Angry and resentful of others Argues with adults Blames others for own mistakes Has few or no friends or has lost friends Is in constant trouble in school Loses temper Spiteful or seeks revenge Touchy or easily annoyed
    To fit this diagnosis, the pattern must last for at least 6 months and must be more than normal childhood misbehavior.

    If you are a rebellious child or teenager and you have not been diagnosed with Conduct Disorder, you are still at risk of being labelled and pathologized. The DSM informs us that "The essential feature of Oppositional Defiant Disorder is a recurrent pattern of negativistic, defiant, disobedient, and hostile behavior towards authority figures that persists for at least 6 months." Unbelievable as this Orwellian, Big Brother text is - it gets worse. If you are under 18 years old and you lose your temper, argue with adults, actively "defy or refuse to comply with the requests or rules of adults", deliberately do things that annoy said adults, blame others for your mistakes or misbehavior - then unquestionably you are a sick little puppy. And who is to make these value judgements? An adult psychologist or psychiatrist or social worker or therapist. And what if you disagree with these authorities? They get annoyed and this is proof positive that you are afflicted with Oppositional Defiant Disorder (ODD). Did anyone mention catch-22?

    What is Conduct Disorder?

    According to DSM-IV criteria, conduct disorder may be diagnosed when a child seriously misbehaves with aggressive or nonaggressive behaviors against people, animals or property that may be characterized as belligerent, destructive, threatening, physically cruel, deceitful, disobedient, or dishonest. This may include stealing, intentional injury, and forced sexual activity. Keep in mind that this behavior disorder consists of a pattern of severe, repetitive acting-out behavior and not of an isolated incident here and there. As stated, conduct disorder is a repetitive and persistent pattern of behavior in which the basic rights of others, or major rules and values of society are violated, as shown by the presence of three (or more) of the following behavior patterns in the past 12 months, with at least one behavior pattern present in the past six months: Aggression to people and animals:
  • Often bullies, threatens, or intimidates others.

A. I am not sure what your daughter's diagnosis is. If it is obsessive compulsive disorder (OCD) or oppositional defiant disorder. The first is 50% boys and 50% girls. The latter is about 90% diagnosed in boys. OCD is treated the same way in kids as adults. It is a biological disorder and needs biological treatment. Luvox, Prozac, and Zoloft all work well. Therapy is very beneficial after the medications start to work. Oppositional Defiant Disorder is usually a diagnosis that is used to describe antisocial children. You are not supposed to make a diagnosis of antisocial personality until the children are older. Oppositional defiant disorder can also be a prequel to borderline personality disorder. Treatment in oppositional defiant disorder should consist of medications and therapy, and should be aggressive. The children start to go down the wrong path in life with this disorderteachers do not like them, other kids do not like them, grades go downand it is very hard to get back on the right track.     Many children and adults with AD/HD also experience other difficulties. It seems that having AD/HD makes it more likely that an individual will also have other difficulties. If a child meets criteria for AD/HD, it may not be enough to prescribe a stimulant get a few checklists and do follow up twice a year. There has been increasing awareness that many adults and children with AD/HD may also meet criteria for one or more other psychiatric diagnoses. (Comorbidity means having two or more diagnosable conditions at the same time) There is some evidence that the incidence of comorbidity is somewhat higher in adults than in children. However, many of the studies looking at the issue of comorbidity are difficult to compare. Studies used different criteria for AD/HD and bipolar disorder, and sometimes got their subjects from different populations. For example, one might expect to see more complex types of AD/HD in specialized hospital clinics than one would see in a door-to-door survey or in a primary care physician’s office. Despite the differing criteria across studies, and the lack of large general population studies of adult AD/HD, there still convincing data that several other psychiatric diagnoses are common among children and adults with AD/HD.

Patient Information Handout Oppositional defiant disorder is defined by the Diagnostic and Statistical Manual of Mental Disorders Oppositional defiant disorder is among the most commonly diagnosed mental health conditions in childhood.     The essential feature of Oppositional Defiant Disorder is a recurrent pattern of negativistic, defiant, disobedient, and hostile behavior toward authority figures in a child that persists for at least 6 months and is characterized by the frequent occurrence of at least four of the following behaviors: losing temper, arguing with adults, actively defying or refusing to comply with the requests or rules of adults, deliberately doing things that will annoy other people, blaming others for his or her own mistakes or misbehavior, being touchy or easily annoyed by others, being angry and resentful, or being spiteful or vindictive. Negativistic and defiant behaviors are expressed by persistent stubbornness, resistance to directions, and unwillingness to compromise, give in, or negotiate with adults or peers. Defiance may also include deliberate or persistent testing of limits, usually by ignoring orders, arguing, and failing to accept blame for misdeeds. Hostility can be directed at adults or peers and is shown by deliberately annoying others or by verbal aggression (usually without the more serious physical aggression seen in Conduct Disorder).

    Definition
    Oppositional defiant disorder (ODD) is a disorder found primarily in children and adolescents. It is characterized by negative, disobedient, or defiant behavior that is worse than the normal "testing" behavior most children display from time to time. Most children go through periods of being difficult, particularly during the period from 18 months to three years, and later during adolescence. These difficult periods are part of the normal developmental process of gaining a stronger sense of individuality and separating from parents. ODD, however, is defiant behavior that lasts longer and is more severe than normal individuation behavior, but is not so extreme that it involves violation of social rules or the rights of others.
