Pediatric Nutrition Guidelines for Discharge Home
When you return home after your transplant, your immune system is still functioning below normal. You may also have increased nutrient needs due to medications or loss of muscle mass. Calories, protein, fluid, vitamins, and minerals are necessary for you to regain your immunity, maintain or achieve your weight goal, strengthen your muscles, and feel better!
Food Safety
  • Allogeneic patients should continue the "Immunosuppressed Patient Diet" until off all immunosuppressive therapy (such as prednisone, FK506, cyclosporine). Autologous patients should continue the diet during the first three months after transplant. When you discontinue the Immunosuppressed Patient Diet continue to use care when purchasing foods, preparing meals and dining out. Situations where you cannot control the cleanliness of the food put you at greater risk for acquiring a food-borne illness. Examples include salad bars, smorgasbords, delicatessens, bulk food bins, free food samples offered in stores. Review the Food Safety Guidelines in the Patient/Caregiver Manual. Continue to avoid herbal and other preparations, since they may contain harmful ingredients or infectious organisms. Herbs are not regulated for purity or for effect on health.

    Pediatrics Nutrition Goal: To familiarize students with the differences in nutritional requirements between adults and children with respect to fluids, electrolytes, calories, carbohydrates, proteins, fats and trace elements. Objectives: 1. Given the age, weight and disease state of a child, calculate the daily maintenance fluids and total daily fluid requirement. 2. List the indications and monitoring parameters (with monitoring frequency) for a child receiving TPN. 3. Given the age, weight and disease state of a child, be able to describe how to initiate and advance TPN with respect to fluids, carbohydrates, protein and fat. 4. Given the age, weight, disease states and TPN order for a pediatric patient, determine if the TPN order is adequate in terms of supplying appropriate calories, protein, fats, electrolytes, minerals, vitamins and trace elements. 5.     In a recent study published by The Lancent Medical Journal , summarized on the CBC website , a clear link has been shown between food additives and preservatives and hyperactivity in children. While confirming something parents have suspected for some time, what is important to note is that hyperactivity in children has been associated with the development of learning difficulties, particularly in regards to reading.

    The Canadian Food Inspection Agency (CFIA) publishes a list of recalls and allergy alerts on their website . With the CFIA typically logging over a dozen alerts each month, parents and caregivers should check the site regularly to ensure that their food supply is free of harmful substances. From a nutritional standpoint, it is highly advisable to limit or remove additives and preservatives in children's diets which, unfortunately, is found in a large number of products at present, often in concealed forms. Innocuous-seeming products such as yogurt, fish sticks and bottled beverages may contain one or more of these harmful chemicals. Check your labels carefully and err on the side of caution when buying your foods.

    • To provide guidelines for the initiation of appropriate nutritional support for pediatric patients in the PICU in a timely manner. To provide guidelines for the provision of an appropriate amount of nutrition given the patient's critical illness and underlying nutritional status aimed at minimizing the catabolic response and improving nutritional status, as appropriate to the patient's condition.
    Nutrition support of the critically ill, post-trauma or post-operative patient continues to develop as a clinical science in light of advances in nutrition, pharmacology and technology. Given the wide diversity in pediatric critical care states, we are challenged to accurately assess the nutritional requirements of our patients and the best mode of nutrition support to optimize their recovery and their nutritional and medical outcomes. Given the complexity of nutritional support in the critically ill child, this website has been organized according to the different categories based on the focus of the nutrition intervention.
