Polio and Post-Polio Syndrome

It attacks your nervous system. In rare cases, polio infection can cause paralysis. Polio vaccination will protect most people for life. The United States and most other countries eradicated polio decades ago, except for rare cases. The disease most commonly affects young children. Poliovirus spreads in human waste. People usually get it from contaminated food or water. Symptoms include fever, tiredness, vomiting, neck stiffness, and leg and arm pain. Most infected people never have symptoms. No treatment will reverse polio paralysis. Moist heat, physical therapy and medicines might ease symptoms. Some people who've had polio develop post-polio syndrome (PPS) years later. Symptoms include tiredness, new muscle weakness and muscle and joint pain. There is no way to prevent or cure PPS.

What is polio?
Polio is a disease caused by a virus. It enters a child's (or adult's) body through the mouth. Sometimes it does not cause serious illness. But sometimes it causes paralysis (can't move arm or leg). It can kill people who get it, usually by paralyzing the muscles that help them breathe. Polio used to be very common in the United States. It paralyzed and killed thousands of people a year before we had a vaccine for it.
Why get vaccinated?
Inactivated Polio Vaccine (IPV) can prevent polio. History: A 1916 polio epidemic in the United States killed 6,000 people and paralyzed 27,000 more. In the early 1950's there were more than 20,000 cases of polio each year. Polio vaccination was begun in 1955. By 1960 the number of cases had dropped to about 3,000, and by 1979 there were only about 10. The success of polio vaccination in the U.S. and other countries sparked a world-wide effort to eliminate polio. Today: No wild polio has been reported in the United States for over 20 years. But the disease is still common in some parts of the world. It would only take one case of polio from another country to bring the disease back if we were not protected by vaccine. If the effort to eliminate the disease from the world is successful, some day we won't need polio vaccine. Until then, we need to keep getting our children vaccinated.

Introduction
Children play outside Khadiga Bent Khiualid Basic School in the city of Kadugli in South Kordofan State in Southern Sudan. Poliomyelitis is a highly infectious viral disease that attacks the nervous system. Children under five years of age are the most vulnerable to the disease, but timely immunization can prevent infection. Approximately one out of every 200-400 children infected will suffer from paralysis and even death. In 1998, the Global Polio Eradication Initiative (GPEI), a partnership of UNICEF, the World Health Organization, Rotary International and the U.S. Centers for Disease Control and Prevention, began spearheading an effort to support country governments to immunize every child against polio until transmission stops and the world is polio-free. Since the GPEI’s momentous launch, nearly five million children, who otherwise would have been paralyzed and incapacitated by polio, are walking, able and symptoms-free. The number of polio cases reported annually has decreased by over 99% – from 350,000 in 1988 to 1,606 cases in 2009. This rapid success has been achieved through a global campaign to immunize to children through mass campaigns, known as National Immunization Days (NIDs). But the work is not complete. Polio remains active in four endemic, four re-established countries, and eleven re-infected countries. In 2010, an outbreak in previously polio-free Tajikistan has accounted for over 62% of new polio cases. Among the four endemic countries, India and Nigeria have shown significant progress with India reducing the number of new cases by more than 95% over last year and Nigeria has seen the new cases decrease by 99% (

THE LATE EFFECTS OF POLIO
Some twenty or thirty years after contracting polio, 70% of people experience new symptoms.
These symptoms may include:
  • new muscle weakness
  • joint and muscle pain
  • breathing problems
The severity of these symptoms may slowly increase.
RESEARCH
Research into the late effects of polio is under way. Some possible causes are:
  • the overuse of polio-weakened muscles leading to deterioration
  • the premature ageing of nerve cells damaged by infection
  • a diminished ability for polio-affected muscles to continue functioning normally
MANAGING THE LATE EFFECTS
Suggestions for the relief of symptoms include:
  • energy conservation
  • weight control
  • posture changes
  • relaxation and stress management

Silver, M.D. , completed her residency at National Rehabilitation Hospital in Washington, DC. A California native, Dr. Silver earned her BS at the University of Califomia/Davis, and her MD at Georgetown University. She interned at the former Framingham Union Hospital. Dr. Silver pursues her interests in musculoskeletal rehabilitation and Post-Polio Syndrome as Director of the new Spaulding Neighborhood Rehabilitation Center in Framingham and is on the faculty of Harvard Medical School Spaulding Rehabilitation Hosptial is an affiliate of The Massachusetts General Hospital and Partners HealthCare System, Inc and is associated with Harvard Medical School, Tufts University School of Medicine and MGH Institute of Health Professions Dr. Silvers has written a new book on Post-Polio Syndrome that will be published early in the year 2000 by Yale University Press. Although it is important to treat the symptoms of PPS, there are many other reasons why polio survivors may become further disabled. This article addresses three major reasons why polio survivors may experience further disability as they age, and how to prevent this from occurring.
