Schizophrenia is characterized by at least 2 of the following symptoms, for at least one month:
  • Delusions
  • Hallucinations
  • Disorganized speech (e.g., frequent derailment or incoherence)
  • Grossly disorganized or catatonic behavior
  • Negative symptoms (e.g., a "flattening" of one's emotions, alogia, avolition; see below)
(Only one symptom is required if delusions are bizarre or hallucinations consist of a voice keeping up a running commentary on the person's behavior or thoughts, or two or more voices conversing with each other.) For a significant portion of the time since the onset of the disturbance, one or more major areas of functioning such as work, interpersonal relations, or self-care are markedly below the level achieved prior to the onset (or when the onset is in childhood or adolescence, failure to achieve expected level of interpersonal, academic, or occupational achievement). Schizoaffective Disorder and Mood Disorder With Psychotic Features have been considered as alternative explanations for the symptoms and have been ruled out. The disturbance must also not be due to the direct physiological effects of use or abuse of a substance (e.g., alcohol, drugs, medications) or a general medical condition. If there is a history of Autistic Disorder or another Pervasive Developmental Disorder, the additional diagnosis of Schizophrenia is made only if prominent delusions or hallucinations are also present for at least a month (or less if successfully treated).

Schizophrenia
Schizophrenia , a term introduced by Bleuler , names a persistent, often chronic and usually serious mental disorder affecting a variety of aspects of behavior, thinking, and emotion. Patients with delusions or hallucinations may be described as psychotic . Thinking may be disconnected and illogical. Peculiar behaviors may be associated with social withdrawal and disinterest.

        The Dopamine Hypothesis of Schizophrenia

        Schizophrenia is a disease that has plagued societies around the world for centuries, although it was not given its formal name until 1911. It is characterized by the presence of positive and negative symptoms. Positive symptoms are so named because of the presence of altered behaviors, such as delusions, hallucinations (usually auditory), extreme emotions, excited motor activity, and incoherent thoughts and speech. In contrast, negative symptoms are described as a lack of behaviors, such as emotion, speech, social interaction, and action. These symptoms are by no means concrete. Not all schizophrenic patients will exhibit all or even a majority of these symptoms, and there is some disagreement in the psychiatric community as to the exact diagnostic criteria. In addition, there is a great deal of debate as to the causes of the disease. While some proposed causes have been proven false, such as bad parenting and poor will power , there are many theories that remain. One of the most famous and most debatable is the dopamine hypothesis. The proposed hypothesis states that the brain of schizophrenic patients produces more dopamine than normal brains. It is this increased dopamine that is believed to be responsible for the symptoms of the disease. However, the is much debate in the scientific community as to the exact mechanism by which altered dopamine levels, especially in the prefrontal cortex, striatum, and limbic system, produce schizophrenia. There is much clinical evidense that provides support for the dopamine hypothesis. The first evidense that dopamine may be involved in schizophrenia came from amphetamine users. Amphetamines work by causing the brain to produce more dopamine and have been shown to produce psychotic-like symptoms.

SOUTH SAN FRANCISCO, CA December 2, 1999 Titan Pharmaceuticals announced encouraging results from preliminary analysis of study ILP3000, the first large scale, controlled Phase III study of the safety and efficacy of the investigational drug Zomaril (iloperidone) in the treatment of schizophrenia.
Zomaril, which has an antagonistic activity on a wide spectrum of brain receptors for dopamine and serotonin, with preferential affinity for 5HT2A serotonin receptors in humans, had previously shown activity against both positive and negative symptoms of schizophrenia, as well as a low liability for inducing extrapyramidal symptoms (movement disorders) in Phase II clinical trials.
Analysis of the ILP3000 results indicates that Zomaril statistically significantly reduced the symptoms of schizophrenia and schizoaffective disorder as measured by the Brief Psychiatric Rating Scale (BPRS) and confirmed the excellent tolerability profile, including the lack of extrapyramidal symptoms.
"We are encouraged by the results of this initial study," stated Dr. Joerg Reinhardt, head of Novartis Pharma Development. "We are well on track in our development of Zomaril as a novel, potential first line therapy for schizophrenia and psychotic disorders."

The Neuroimaging Research Group of the Department of Psychiatry at UMC Utrecht studies structural and functional brain maturation in humans throughout life, in health and diseases such as schizophrenia, bipolar disorder, depression, ADHD, and autism.
Studies are aimed at finding genetic and environmental markers of variance in brain structure and function over time. We aim to unravel (patho)-physiology of brain plasticity of human brain throughout life.
We develop and implement Magnetic Resonance Imaging (MRI) brain scan protocols and processing pipelines. Scans are being acquired for this purpose on the 1.5T, 3T and 7T scanners at our hospital on a daily basis. Processing pipelines exist for volume quantification, DTI, resting-state fMRI, BOLD fMRI, TMS-fMRI, and functional DTI.
Cohorts of healthy subjects, patients with psychiatric disorders, and their family members, including monozygotic and dizygotic twins and singleton siblings are followed longitudinally at a 2-5 year interval. To date, the continuously growing database consists of over 3000 MRI brain scans.
The Neuroimaging Research Group is based in the Psychiatry Department, Neuroscience Division, of the University Medical Center Utrecht (UMCU), The Netherlands. It is also part of the Rudolf Magnus Institute of Neuroscience (RMI), which encompasses many research groups from the UMCU as well as from the Utrecht University.

Introduction
Numerous studies have established the familial aggregation of schizophrenia and that familial factors that predispose to schizophrenia also predispose to other schizophrenia related disorders. Kraepelin proposed that schizophrenia and manic-depressive psychosis were two distinct disorders but since then with abundance of literature on this subject pouring in, the theory is being challenged quite frequently. The number of studies which support the Kraepelinian view of psychosis exceed those which do not. The proponents of continuum theory propose a continuum from unipolar to bipolar disorder to schizoaffective psychosis and right up to schizophrenia. Another hypothesis proposed is that schizophrenia and bipolar affective disorder are two distinct entities and that unipolar and schizoaffective disorders are expressed in the relatives of patients in both the categories (Gershon et al, 1988).
Family Studies
Rudin (1916) reported that although the risk for schizophrenia for an individual was high (6.2%) if they had a close relative with schizophrenia, it was higher if the ill relative had a non-schizophrenic (mostly affective) psychosis (8.2%) or alcoholism (7.8%). He also found almost equal numbers of relatives with affective disorder and relatives with schizophrenia in the families of his schizophrenic probands. On the other hand, among the psychotic relatives of Odegaard's (1972) manic depressive probands, 19% were diagnosed as schizophrenic, indicating that the co-occurrence may run in both directions.
