        By Robert Burton , M.D. D own in the dumps? Want to curl up in bed rather than go hang gliding? Are you eating lots of candy and fruitcake? Gaining weight? You bet: It's cold and dark out. More than just a season, winter has become a metaphor for the dark side of life. Most of us accept the winter blahs as normal and wait for spring. But for some, seasonal doldrums can be a real problem. My mother, a tradition-confused West Coast Jew, might have been onto something. She insisted on having a large Christmas tree in the living room, but she refused the usual panoply of ghastly colors. Instead she used exclusively white bulbs. "Colored lights are for the goyim," she would say with the slyest of smiles. When pressed, she would add, "Bright white is better for your spirits." (Another Jewish tradition justifying any peculiar taste on the basis of health reasons). But was my mother right? Were the bright white Christmas tree bulbs therapeutic? Would votive candles have done the job? Or a trip to Florida? Or spending the winter in a brightly lit casino? Are TV and computer monitors evolutionary answers for the winter blahs? I don't doubt that we get bummed out at this time of year, but is this a specific biological condition or a more metaphysical malaise?

A patient information handout on seasonal affective disorder , written by the authors of this article, is provided on page 1351. D epressive episodes are a primary public health problem and one of the most common psychiatric conditions in patients seeing family physicians, with a lifetime prevalence of 17.1 percent in the general population. Some of these mood disturbances follow regular seasonal patterns. These seasonal mood patterns have been termed seasonal affective disorders (SADs). Description The Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) describes SAD not as a separate mood disorder but as a "specifier," referring to the seasonal pattern of major depressive episodes that can occur within major depressive and bipolar disorders. Table 1 summarizes the DSM-IV criteria for a major depressive episode.

Seasonal affective disorder
Seasonal affective disorder (SAD) refers to episodes of depression that occur every year during fall or winter. Symptoms improve in spring and summer.
Causes
The disorder may begin in adolescence or early adulthood. Like other forms of depression, it occurs more often in women than in men. Most people with the "winter blahs" or "cabin fever" do not have SAD. People who live in places with long winter nights are not necessarily more likely to have SAD. The cause of SAD is not known, but it is thought to be related to many factors, including:
  • Ambient light Body temperature Hormone regulation
A rare form occurs in the summer.
Symptoms
Symptoms usually build up gradually in the late autumn and winter months.
  • Afternoon slumps with decreased energy and concentration Increased appetite with weight gain (weight loss is more typical of other forms of depression) Increased sleep and excessive daytime sleepiness (problems sleeping are more typical of other forms of depression) Lack of energy and loss of interest in work or other activities Slow, sluggish

What is seasonal affective disorder?
Seasonal affective disorder (also called SAD) is a type of depression that is triggered by the seasons of the year. The most common type of SAD is called winter-onset depression. Symptoms usually begin in late fall or early winter and go away by summer. A much less common type of SAD, known as summer-onset depression, usually begins in the late spring or early summer and goes away by winter. SAD may be related to changes in the amount of daylight during different times of the year. Return to top
How common is SAD?
Between 4% and 6% of people in the United States suffer from SAD. Another 10% to 20% may experience a mild form of winter-onset SAD. SAD is more common in women than in men. Although some children and teenagers get SAD, it usually doesn't start in people younger than 20 years of age. For adults, the risk of SAD decreases as they get older. Winter-onset SAD is more common in northern regions, where the winter season is typically longer and more harsh. Return to top
