Graves' Hyperthyroid Patients
with Active Thyroid Eye disease. The presence of thyroid tissue is felt to be a stimulus to promote thyroid eye disease. Therefore removing all thyroid tissue has been shown to decrease eye disease. Total thyroidiectomy still leaves a small amount of thyroid tissue behind. 55 patients with Grave's hyperthyroidism and mild or moderate eye changes (swelling, double vision, protrusion of the eyes). All had thyroidectomy, and 16 of the 55 had Radioiodine ablation of the residual thyroid tissue after the surgery. All received T4 therapy. Clinical eye scoring was done at the time of surgery and at 6, 12, and 24 months. Results - 31 had active eye disease at the time of the surgery. There was a 61% drop in active eye disease by 6 months, and 71% were inactive at 24 months. 38% of inactive eye disease patients became active during 24 month follow-up. Of the 39 patients treated by thyroidectomy alone the eye disease was still active in 46%. However, all 16 patients treated by TX, and Radioiodine had inactive eye disease by 24 months.

THYROID EYE DISEASE What is thyroid eye disease? What causes thyroid eye disease? Does everyone with an overactive thyroid develop thyroid eye disease? Are there any other symptoms? ... Will my eyes return to normal? What is thyroid eye disease? Thyroid eye disease often occurs in people who develop an overactive thyroid gland . Swelling of the muscles and other tissues in the orbits causes the eyes to become pushed forward and more prominent. The eyes often take on a more staring appearance. In more severe cases the swelling may cause stiffness of the muscles which move the eyes. This can cause a "squint" to develop and may result in double vision. Occasionally the swelling behind the eyeball may press on the nerve from the eye to the brain and distrupt vision. Thyroid eye disease is also called thyroid opthalmopathy, Graves’ eye disease or dysthyroid eye disease. What causes thyroid eye disease? Most people with thyroid eye disease have or had or will subsequently develop an overactive thyroid gland. In 20% the thyroid eye disease develops in people who do not have an overactive thyroid at the time (may subsequently develop this years later). In 40% thyroid eye disease occurs whilst the thyroid is overactive and in 40% can occur years after the overactive thyroid has been treated successfully. Overactivity of the thyroid gland is usually caused by an "autoimmune condition" This means that cells which normally protect the body from infection develop a "fault" and begin to recognise the thyroid gland as foreign material and attack it. This stimulates the thyroid gland to produce extra thyroid hormones. The attacking process may spill over to the cells behind the eye causing them to swell. It is not yet known why cells develop the fault that causes them to attack the thyroid gland or why only some patients with overactivity of the thyroid develop thyroid eye disease. Thyroid eye disease does appear to be more common in smokers.

The thyroid gland
The thyroid is part of the endocrine system, the system of organs that secrete hormones into the blood. A butterfly-shaped gland wrapped around the front of the larynx, it is the largest endocrine gland and the first to appear in the fetus during pregnancy. Its purpose is to synthesize T4 and T3 from iodine. Follicles (round sacs) in the thyroid are filled with thyroglobulin, a thyroid protein. When dietary iodine enters the thyroid, it travels to the follicles, where T4 and T3 are made. The thyroid hormones are first stored in the thyroglobulin and then released into the bloodstream. A complex system of proteins and other molecules carries the thyroid hormones to the cells. When the thyroid is underproducing, the action of

It is one of your endocrine glands, which make hormones. The thyroid helps set your metabolism - how your body gets energy from the foods you eat. Millions of people in the U.S. have thyroid diseases. Most of them are women. If you have a thyroid disease, your body uses energy more slowly or quickly than it should. A thyroid gland that is not active enough, called hypothyroidism, is far more common. It can make you gain weight, feel fatigued and have difficulty dealing with cold temperatures. If your thyroid is too active, it makes more thyroid hormones than your body needs. That condition is hyperthyroidism. Too much thyroid hormone can make you lose weight, speed up your heart rate and make you very sensitive to heat. There are many causes for both conditions. Treatment involves trying to reset your body's metabolism to a normal rate.
