Although no specific data exists regarding the social impact of temporamandibular joint disorders, TMJ is estimated to account for as much as thirty billion dollars a year in lost productivity. Americans lose 550 million work days every year due to symptoms associated with TMJ, with facial pain and headache being the most common complaints. Accordingly, analgesics directed at these symptoms are among the top selling over the counter medicines in our society. Despite the lack specific data regarding the incidence of these disorders, much has been learned over the last decade regarding specific pathology and therapy. Generic terms such as TMJ Syndrome, or Myofascial Pain Dysfunction Syndrome (MPD Syndrome) are generally outdated. TMJ Syndrome Overview
Temporomandibular joint (TMJ) syndrome is pain in the jaw joint that can be caused by a variety of medical problems. The TMJ connects the lower jaw (mandible) to the skull (temporal bone) in front of the ear. Certain facial muscles control chewing. Problems in this area can cause head and neck pain, facial pain, ear pain, headaches, a jaw that is locked in position or difficult to open, problems biting, and jaw clicking or popping sounds when you bite.

Phase I: Reversible treatment may include patient education, day and/or nighttime splints, diagnostic and trigger point injections, physical therapy, inflammatory and pain medication. Initial treatment objectives are designed to reduce muscle spasm, swelling in the jaw joints, related pain, and restore normal function and insure a restful sleep pattern.
Phase II: Non-reversible treatment may include occulsal adjustment (bite), orthodontic treatment, restorative measures including crowns, and/or bridges, partials and occasionally surgery. Phase II objectives are designed to correct symptoms and lack of normal function that persists after the initial treatment procedures.

Open your jaw all the way and shut it. This simple movement would not be possible without the Temporo-Mandibular Joint (TMJ). It connects the temporal bone (the bone that forms the side of the skull) and the mandible (the lower jaw). Even though it is only a small disc of cartilage, it separates the bones so that the mandible may slide easily whenever you talk, swallow, chew, kiss, etc. Therefore, damage to this complex, triangular structure in front of your ear, can cause considerable discomfort. Where is the Temporo-Mandibular Joint? You can locate this joint by putting your finger on the triangular structure in front of your ear. Then move your finger just slightly forward and press firmly while you open your jaw all the way and close it. You can also feel the joint motion in your ear canal. How does the Temporo-Mandibular Joint work?

Understanding TMJ
Your jaw joint which holds your lower jaw in place is suspended beneath your skull by an intricate system of muscles and tendons. The jaw joints also known as the temporomandibular joints (TMJ) are some of the most complex in the body. The jaw joint, surrounding muscles, and the fit of your teeth are closely interrelated, each affecting the other as you make every day jaw movements such speaking and chewing. The way your teeth fit together is called occlusion. When your teeth are not in proper relation to each other and to your jaw joints, the jaw automatically shifts to a new position in an attempt to compensate for the misalignment of your teeth known as malocclusion. It can be seen from the diagram how a misaligned TMJ can affect blood vessels, nerves and muscles, causing severe headache or ear pain. Now also imagine what can happen to the rest of the body. If the jaw is out of alignment, other skeletal structures (such as the neck and spine) can become involved, which can result in other joints, nerves, muscles and blood vessels being affected. As nature tries to compensate for one difficulty, others are likely to arise. Structural stress from a misaligned TMJ is the worst kind of physical stress because there is no way the body can find relief for it. An injured limb can be rested, but it is impossible to leave the TMJ inactive. Without realizing it, we swallow hundreds of times a day, and at night we may clench or grind our teeth. Every time those teeth meet in incorrect occlusion, there is stress. To compensate for it, the jaw muscles begin to hold a tense unnatural position; and before long, the tension travels by chain reaction throughout the body.

      If you are suffering from headaches, all too often you may be diagnosed with general terms such as tension headache, migraine, or TMJ pain without the true cause being identified. Often the cause is trigger points in one or more muscles, but the diagnosing practitioner is unfamiliar with trigger points. Trigger points can play a very large role in most types of chronic and acute headache, as well as temporomandibular joint (TMJ) problems, which means you may be able to get a great deal of relief, or even complete relief, by working on trgger points and eliminating perpetuating factors. Your headache, migraine, and TMJ pain is likely treatable. The sooner you start doing work on them or having a practitioner treat them, the sooner you will get relief. This is important, since untreated pain can create an escalating cycle that makes the pain more chronic and more resistant to treatment. When pressed, trigger points are usually very tender. The sustained contraction of the fibril probably leads to the release of sensitizing neurochemicals which are body substances that affect the nervous system. This produces the pain that is felt when the trigger point is pressed. Pain intensity levels can vary depending on the amount of stress placed on the muscles. The intensity of pain can also vary in response to flare-ups of any of the other perpetuating factors, such as emotional factors, illnesses, and insomnia. Healthy muscles usually do not contain knots or tight bands, are not tender to pressure, and, when not in use, feel soft and pliable to the touch, not like the hard and dense muscles found in people with chronic pain.
