As part of the article, Dynamic Magnetic Resonance Imaging Technique for the Study of the Temporomandibular Joint by Drs Yunn-Jy Chen, Luigi M. Gallo, Dieter Meier, and Sandro Palla (J Orofac Pain 2000;14:65-73), website visitors can now view a series of dynamic MRI of temporomandibular joint movements. Physical Therapy Corner: The Temporomandibular Joint
by admin last modified The temporomandibular joints (TMJ) are among some of the most frequently used joints in the body, allowing us to talk, chew, yawn, swallow and sneeze. The TMJ are located on each side of the cranium. To find your TMJ, place your fingers in front of each ear and open your mouth, you will feel an indentation beneath your finger. The two bones that form the TMJ are the mandible (jaw) located inferiorly, and the temporal bone of the skull (located more superiorly) (see Figure 1). A disc that is connected to the capsule divides the joint cavity into inferior and superior spaces. (see Figure 2) Temporomandibular joint dysfunction (TMD) is often overlooked. Some of the more common symptoms include clicking or popping with opening or closing of the mouth, pain around the jaw joints, locking of the jaw, headaches and an improper bite (teeth do not fit together properly) (see Figure 3). Most often, TMD can be treated conservatively. However, it is important that a qualified clinician such as a physical therapist, dentist, or orthopedist is consulted for an accurate diagnosis since many conditions can mimic the signs and symptoms common to TMD.

Symptoms include head, neck, and even back aches. Patients with TMJ problems may also complain of earaches, bouts of deafness, or ringing in the ears. Clicking and popping can occur with chewing. Some patients experience dizziness, chronic fatigue, and difficulty swallowing.
TMJ dysfunction usually starts with jaw misalignment from some source. Finding that source is the difficult part of treating TMJ pain. When there's an imbalance in the jaw, the muscles in the neck and shoulders try to compensate, causing pains and headaches that can't be relieved until the jaw is realigned. Likewise, the reverse can be true; if the muscles in the neck and shoulders are in spasm, the jaw can be pulled out of alignment.
Poor posture, including the way you hold your head, carry your torso, or even the way you stand, can cause muscle imbalance that affects jaw motion as well. In fact, anything that may disturb normal muscle function, such as ligaments, nerve, or even disk disease can cause TMJ pain.
If someone believes the pain is starting from their jaw, they are likely to go to their dentist for help. His first priority will be to ensure there is no dental disease or other urgent situation. Having ruled this out, it is likely he will want to correct jaw alignment with the goal of restoring normal function and reducing muscle spasm.

TMJ is a term used to refer to temporomandibular joint disorder. It is also called TMD, TMJ dysfunction, or TMJ syndrome. The temporomandibular joint is very complex. It has a hinge motion but also has sliding motions. The term TMJ disorder can refer to problems with the joint itself, such as arthritis or inflammation, but often actually refers to disorders of the muscles: spasms, tenderness, etc. The disorder and resultant dysfunction can cause significant pain that can be confined to the jaw area or can radiate into headaches of varying degrees of severity. There are various treatment philosophies. They are based on ideas about how the jaw ought to function, and usually the treatment is centered around restoring jaw function to that ideal, which relieves the pain and discomfort. Successful TMJ treatment requires advanced training in occlusion beyond what most dental schools offer, and I recommend that, if you have this disorder, you seek the services of a dentist with this advanced training. Two respected institutes that offer advanced training in occlusion are: The L.D. Pankey Institute and The Las Vegas Institute for Advanced Dental Studies. The treatment philosophies of these two institutes are very different, but dentists trained at either one have success in the treatments they offer. Expert cosmetic dentists tend to understand this area of dentistry very well, and many of the dentists we recommend, in addition to their cosmetic dental training, have training at either the Las Vegas Institute or the L.D.
