Personalized Medicine
Personalized Medicine: The Background
Personalized medicine is an extension of traditional approaches to understanding and treating illness. Since the beginning of the study of medicine, physicians have employed evidence found through observation to make a diagnosis or to prescribe treatment. In the past, this was presumably tailored to each individual, but personalized medicine makes treatment more specific. In the modern conception of personalized medicine, the tools provided to the physician are more precise, probing not just the obvious, such as a tumor on a mammogram or cells under a microscope, but the very molecular makeup of each patient. Looking at the patient on this level helps the physician get a profile of the patient’s genetic distinction, or mapping. By investigating this genetic mapping, medical professionals are then able to profile patients, and use the found information to plan out a course of treatment that is much more in step with the way their body works. Genomic medicine and personalized medicine use genetic information to prevent or treat disease in adults or their children.

Optimizing a Unimodal Response Function for Binary Variables
Janis Hardwick Quentin F. Stout
University of Michigan Abstract : In some dose response settings in clinical trials, there are completing failure modes where low doses do not elicit a positive response, while overly large doses elicit a toxic response. Typically this occurs in trials known as phase I/II trials. In such settings, often the dose response curve which plots dose vs. success (efficacious and non-toxic), is unimodal, i.e., it is increasing and then decreasing. This then leads to the problem of finding the mode (highest point) of the curve. In this paper, several sampling designs are examined for the problem of optimizing a response function from a set of Bernoulli populations, where the population means are assumed to have a strict unimodal structure. The designs are evaluated both on their efficiency in identifying a good population at the end of the experiment, and in their efficiency in sampling from good populations during the trial. A new design, that adapts multiarm bandit approaches to this unimodal structure, is shown to be superior to the designs previously proposed. These earlier designs were based on Polya urns, stochastic approximation, and up and down rules (random walks, Markov chains). The bandit design utilizes approximate Gittins indices and shape constrained regression to model the dependencies between the arms. Note that our design, and some of the others, are nonparametric, so they are quite flexible and do not make strong assumptions about the shape of the curve.

PERSONALIZED MEDICINE (PHARMACOGENOMICS)
Why do people vary in their responses to prescribed medications, both with respect to how well it works and adverse reactions to it? Researchers think the answer lies in our genes. Scientists, physicians and the pharmaceutical industry are actively developing ways to customize medical treatments to suit our unique genetic signatures. The study of how genetic variations interface with drug response and disease risk is called pharmacogenomics.

What is pharmacogenomics?
Pharmacogenomics is the study of how an individual's genetic inheritance affects the body's response to drugs. The term comes from the words pharmacology and genomics and is thus the intersection of pharmaceuticals and genetics.

ONE SIZE DOES NOT FIT ALL:
THE PROMISE OF PHARMACOGENOMICS
Adverse Drug Reaction. These three simple words convey little of the horror of a severe negative reaction to a prescribed drug. But such negative reactions can nonetheless occur. A 1998 study of hospitalized patients published in the Journal of the American Medical Association reported that in 1994, adverse drug reactions accounted for more than 2.2 million serious cases and over 100,000 deaths, making adverse drug reactions (ADRs) one of the leading causes of hospitalization and death in the United States. Currently, there is no simple way to determine whether people will respond well, badly, or not at all to a medication; therefore, pharmaceutical companies are limited to developing drugs using a "one size fits all" system. This system allows for the development of drugs to which the "average" patient will respond. But, as the statistics above show, one size does NOT fit all, sometimes with devastating results. What is needed is a way to solve the problem of ADRs before they happen. The solution is in sight though, and it is called pharmacogenomics.

Unsourced material may be challenged and removed (May 2007) The terms pharmacogenomics and pharmacogenetics tend to be used interchangeably, and a precise, consensus definition of either remains elusive. Pharmacogenetics is generally regarded as the study or clinical testing of genetic variation that gives rise to differing response to drugs , while pharmacogenomics is the broader application of genomic technologies to new drug discovery and further characterization of older drugs. Pharmacogenetics refers to genetic differences in metabolic pathways which can affect individual responses to drugs, both in terms of therapeutic effect as well as adverse effects.

The Project is part of the Pharmacogenetics Research Network and Knowledgebase. Information about the entire Network can be found at PharmGKB . Pharmacogenetics is the study of the genetic basis for variation in response to drugs. Membrane transporters play a major role in drug response in two ways. First, many drugs work by affecting function of transporters. Second, transporters determine the level of drugs within the body by mediating drug absorption, distribution and elimination. Transporters are also determinants of drug resistance. The goal of the UCSF PMT Project is to understand the genetic basis for variation in response for drugs that interact with membrane transporters. Support for the bioinformatics activities of the PMT project also comes from the NIH National Center for Research Resources (grant P41 RR01081).
